We offer independent and supportive housing services throughout Santa Clara County. These programs give clients a safe place to call home鈥攁nd help them practice living more independently whenever possible.

Many clients without stable housing have been through difficult circumstances, such as getting a serious medical diagnosis, recovering from substance use, losing loved ones, and experiencing trauma. These situations require unique housing solutions. Our housing options are flexible to meet clients鈥 specific needs.听

Our team offers customized treatment and case management, which may include:

  • Crisis intervention
  • Assistance with daily living skills听
  • Employment support听
  • Medication support services
  • Rehabilitation and therapy groups
  • Transportation assistance

What to Expect:

As a resident in independent or supportive housing, you will:

  • 1.

    Work with a housing specialist who helps you build relationships with your property manager and understand your rights and responsibilities as a renter.

  • 2.

    Receive assistance and care for physical health, mental health, and substance use, and help with other tasks, like applying for Social Security benefits or gaining employment.

  • 3.

    Have access to a team of mental health rehab specialists, psychiatrists, substance use specialists, and employment case managers as appropriate.

Who Is Eligible?

To access housing, you must fit these criteria:

  • You are a client at Momentum and actively working your treatment program. We do not offer housing to the public.
  • You are 18 years old or older.
  • You have active Medi-Cal, Medicare, or no insurance. We cannot accept private insurance.
  • You are not in need of drug or alcohol detox services.
  • You are in a crisis that is the result of behavioral health distress.

Independent housing includes food, a washer and dryer, utilities, cable, internet, and a desktop computer

You also receive on-site clinical care as part of a personalized treatment plan. Your care may include one or more of these on-site services:

  • Case management
  • Crisis intervention
  • Employment services听
  • Group or individual therapy
  • Medication support services
  • Substance use treatment

You also practice independent living skills, such as:

  • Computer skills听
  • Housekeeping
  • Money management
  • Nutrition and meal preparation听
  • Personal hygiene听
  • Shopping
  • Using public transportation

Supportive housing serves clients who need stable housing but do not need on-site clinical care. In supportive housing, you are responsible for your own food and utilities. You have access to wraparound services at 新澳门六合彩官方鈥檚 offices, which may include:

  • Mental health rehabilitation
  • Addiction counseling
  • Appointments with a primary care physician and/or psychiatrist
  • Employment services

Step by Step

  • 1.

    You start the process to enter our housing programs with an interview. A case manager assesses whether you meet income requirements, which are based on a sliding scale. If you have no income, you can self-declare.

  • 2.

    If we have more applications than openings, we place you on a waiting list based on your application date.

  • 3.

    After you are admitted to housing, you move into your space, meet your neighbors, and have regular meetings with your housing specialist.

  • 4.

    You and your housing specialist decide how long you need to stay in the program.

  • 5.

    Your housing specialist helps you find suitable housing when you are ready to step down from this level of care.

Recovering from addiction, a mental health crisis, or complex trauma is challenging for anyone, especially for clients who are unhoused. Safe, stable housing is key for people working through the recovery process. Our housing programs allow clients to focus on their treatment plan, rather than where they鈥檙e sleeping at night or where their next meal is coming from.

Having a secure place to live also helps our clients build a healthy sense of self. This means they can work toward goals they set for themselves and become responsible tenants and members of their communities.

Day in the Life

Freedom of choice is key to client success. Both our independent and supportive housing models allow choices in housing and access to services.听

As a client in our housing program, you must be actively working through your treatment plan. This may include taking medications, traveling to appointments on public transportation, participating in groups if you鈥檙e not working, and attending house meetings.

You are also expected to be a good neighbor. This includes respecting those around you, communicating in healthy ways during conflicts, having appropriate boundaries, and maintaining your living space.听
